2. How to fill the form

The form first requires the student to select the course they want to comment on, then the student can comment on each of the following items. The form can accept up to 10 comments numbered 1 through 10:

Comment 1: Instructor (list his name first)

Brian Linard(Section 001) or Kris Miller(Sections 002/003)

Comment 2: TA (list his/her name first)

Comment 3: In-Lab exercises

Comment 4: Homework

Comment 5: Quizzes

Comment 6: The course overall -- please grade the course (A-F), and explain.

Comment 7: Tell us your major and how your studies are going so far.

Comment 8: If you are a CS/EE/CE/IS major, are you seriously thinking of

switching out of the CS/EE/CE/IS major (or have you already done
